Our multidisciplinary Assertive Community Treatment (ACT) teams offer a full range of services for people with severe mental illness who may be transient and have difficulty connecting to care.

 

How we help

 

We meet you where you’re at and offer the support you need, including:  

  • Employment services 

  • Housing case management 

  • 24/7 crisis support 

  • Peer support

  • Relationship building and support groups 

  • Nursing and psychiatric care 

Who is eligible?

 

Anyone 18 years and older with severe and persistent mental illness, including schizophrenia and psychotic disorders. You must have had at least 4 previous hospitalizations and/or ER visits and a psychiatric evaluation within the past 6 months.

Brooklyn Connected Care provides services regardless of a person’s ability to pay – this includes the underserved, those with little to no income, those that are insured as well as those who are uninsured or on Medicaid. Brooklyn Connected Care also provides care to active-duty military personnel and veterans.
